Strongly Preferred: PGY-2 Specialized residency in Oncology Pharmacy Practice or equivalent experience. Completion of PGY-1 Pharmacy residency or equivalent experience. QUALIFICATIONS Required: Currently licensed as a pharmacist with Arizona State Board of Pharmacy. Also responsible for the education of pharmacy students, interns and visiting PGY-1 residents when applicable. Providing education regarding medication therapy to providers, nurses, staff pharmacists, other disciplines as well as patients-caregivers when appropriate. Developing patient care plans in concert with the medical team and recommending therapy or alternative approaches based on interpretation of relevant medical literature and clinical experience Participating in facility specific Pharmacy and Therapeutic Committee and other Medical Staff Committee meetings and also representing facility on System Committees as appropriate. The Oncology clinical pharmacist at BTMC will provide comprehensive clinical pharmacy services through coordination and management of the medication therapy process by: Participating in direct patient care with a medical team functioning as a therapeutic consultant confirming the rational and appropriate use of medications and serving as a content expert in clinical pharmacology, pharmacokinetics, disease state management pertinent to the oncology population (antimicrobial use, nutrition support, pain management) Assisting with therapeutic decisions such as chemotherapy regimen selection, appropriate dosing, symptom management and therapeutic monitoring. The successful candidate will possess advanced training and experience in Oncology practice. THUNDERBIRD CLINICAL PHARMACIST-ONCOLOGY This individual will be responsible for provision of clinical pharmacy services to care for oncology patients as a member of a multidisciplinary team at Banner Thunderbird Medical Center.
